Forms, Templates and Resources

USDA Forest Service Federal Financial Assistance Forms and Templates. To download any of the files listed below, click on the files title and the file will be downloaded to your browser's default download folder. To access the downloaded file, navigate to your browser's download folder. NOTE: All completed forms uploaded to the Grant Portal MUST be Adobe PDF formatted files. Forms in other formats such as Word Documents and Spreadsheets must be saved-as or converted to an Adobe PDF file before uploading to your online application.

Resources

To view a resource file, click the title of the link of interest. Depending on your browser's settings, the resource file will either open in a new browser window or will be downloaded to your browser's default download folder. If downloaded to your browser's download folder, you'll need to navigate to your browser's download folder and open the file there. Videos will display in a new browser window.

Templates

To download a template, click the title of the template of interest. The template file will be downloaded to your browser's default download folder. To open the downloaded file, navigate to your browser's download folder and select it from the list of downloaded files. The file will then open in your computer's default file viewer for the file's type (i.e. PDF, Work, Excel, etc.).

  • Project Narrative Template (MS Word)

    You MUST use this Word document template as the starting point for your project narrative. Your completed narrative must be no more than 7 pages in length. Once you complete detailing each section contained in the narrative, save it as an Adobe .pdf document and upload it to the Project Narrative workflow application step in your online Application Dashboard.

  • Simple Budget Table Template (MS Excel)

    This budget template is provided as a convenience for those in need of a simple budget. Large projects will require extensive details and you may use any budget format of your choosing.

Forms

To download a form, click the title of the form of interest. The form will be downloaded to your browser's default download folder. To open the downloaded file, navigate to your browser's download folder and select it from the list of downloaded files. The file will then open in your computer's default file viewer for the file's type (i.e. PDF, Work, Excel, etc.).

NOT all forms listed below are required for your online application. Only the specific forms requested by the application workflow steps are required for your online application. Some of the forms listed below may be requested from you at a later date should your application be selected by the review team. Do NOT upload unsolicited forms to your online application.

  • SF-424

    SF-424 - Application for Federal Assistance. The applicant should download this fillable Acrobat Form and complete it with the requested information. Once completed, export the Adobe Form as a Word document (.doc or .docx) and then, from within Word, save it as a standard Adobe .pdf document. These steps are needed to convert the Acrobat Form to a standard Acrobat .pdf document. Then, upload the Acrobat .pdf document to the FS-424 workflow step in your Application Dashboard.
